The National Hurricane Center named Katia a tropical storm today, August 30, 2011, at 5 a.m.EDT.As newborn Katia speeds west-northwest the current track projects it moving north of the Leeward Islands on the weekend.Because wind shear is light and sea surface temperatures are warm in the area where Katia is headed, the National Hurricane Center forecasts strengthening into a hurricane.

On August 30, at 5 a.m.EDT, Katia's maximum sustained winds were near 40 mph.Additional strengthening is forecast by the National Hurricane Center.Katia is currently a compact tropical storm, only 70 miles in diameter, compared with Irene that was as large as 600 miles in diameter.

Katia was located near latitude 11.8 North and longitude 31.7 West and moving west-northwest near 17 mph (28 kmh).It is expected to continue in this direction for the next two days and speed up.Katia's estimated minimum central pressure is 1006 millibars.

Research Activities, September 2011: Feature Story: The challenge of prioritizing care for complex patients Research Activities, September 2011: Feature Story: The challenge of prioritizing care for complex patientsA patient with only one chronic condition walks into an exam room.This is becoming less common than it used to be.Optimizing health care and ultimately the health of Americans is getting harder as patients increasingly have more than one chronic condition: diabetes, depression, heart disease, and more.

"We're not surprised to have patients with five or more chronic conditions anymore," says Cynthia Boyd, M.D., of the Johns Hopkins University School of Medicine's Division of Geriatric Medicine and Gerontology, who is studying how complex health status is a*sociated with treatment burden."We face enormous challenges trying to figure out which things are most important," says Dr.Boyd.

Researchers are looking at how to prioritize treatment and preventive care for these patients.For example, how do you decide which medications to prescribe when treating patients with multiple conditions when the medicines may have dangerous interactions?And, what are the harms and benefits of preventive tests that may unnecessarily burden patients with multiple chronic conditions who are already juggling numerous drugs and doctors' visits?

The care costs for these patients are also a concern.Treating patients with several chronic conditions can cost as much as seven times more than treating patients with only one chronic illness.Among Medicare beneficiaries, 66 percent of spending is for those with five or more chronic conditions.Since 2008, the Agency for Healthcare Research and Quality (AHRQ) has awarded 47 grants to researchers looking for better ways to study and provide care for complex patients.

The perfect storm

Maureen Smith, M.D., refers to the growing number of complex patients as "a perfect storm." And it's a fast moving one."We don't have time to look for the perfect answer, but we do have the opportunity to look into treatments that will work in the real world," says Dr.Smith, director of the Health Innovation Program at the University of Wisconsin School of Medicine and Public Health.

Dr.Smith is examining how tight adherence to diabetes treatments may affect patients with diabetes who also have chronic kidney disease and congestive heart failure.Her work aims to help clinicians prioritize which of these issues to address first in caring for complex patients."Our project examined whether tight adherence to diabetes guidelines, particularly tight control of blood sugar, is indicated for all patients with diabetes," Dr.Smith told Research Activities."We found that diabetes patients with very tight control of their blood sugar (hemoglobin A1c <5.5%) had a higher risk of hospitalization, emergency department visits, or death, and this risk was significantly increased in more complex patients."

David M.Kent, M.D., of Tufts Medical Center, has spent the past 10 years studying this real world as he explores better ways to do subgroup analysis."Because patients have too many different characteristics that can potentially alter the risks and benefits of therapy to consider each one separately, we advocate a risk-modeling approach to the interpretation of clinical trials," Dr.Kent told Research Activities."In contrast to the conventional one-variable-at-a-time approach to subgroup analysis, this approach seeks to describe the dimensions that determine the likelihood that a patient might benefit or be harmed by therapy."

The role of prevention

By studying the benefits and harms of preventive interventions in elderly patients with cognitive impairment, Greg A.Sachs, M.D., hopes to improve decisionmaking.All of his patients are by definition "complex patients."

Early in Dr.Sachs's career, he received a fax from a pharmacist wondering why a woman with diabetes in the nursing home wasn't on a statin for her ch*lesterol.The patient was 95 and had advanced dementia, among other conditions."She was dead by the time the fax arrived," says Dr.Sachs, of the Indiana University Division of General Internal Medicine and Geriatrics and Regenstrief Institute, Inc.He's even had to explain why an 85-year-old patient with dementia and multiple other conditions shouldn't be subject to a colonoscopy."When patients don't benefit from screenings, it's not good medicine and it's not smart spending," he told Research Activities.

For clinicians like Dr.Boyd of Johns Hopkins University, who work with complex patients every day, one of the most difficult things to convey can be uncertainty.Yet, she says, "I have to communicate what I know and don't know.We need to balance both patient-centered care and evidence-based guidelines.We can't think about one without the other."

In an article published in the Journal of the American Medical Association in 2005, Dr.Boyd and her coauthors applied evidence-based guidelines to a hypothetical elderly woman with five common chronic diseases.They found she would need at least 12 medications (costing her $406 per month) and a complicated nonpharmacological regimen to manage her conditions.Creating a care management plan that is achievable and not burdensome requires a large dose of reality."If we try to do everything, it will be overwhelming for our patients and not even in line with their goals," says Dr.Boyd."I'm constantly trying to figure out the best way to energize and motivate my patients for the self-management, treatment, and interventions that are the highest priorities for their well-being."

Caring for the wh*le patient

"We need to shift from caring for each of a patient's conditions to caring for the patient as a wh*le," says Dr.Victor Montori of the Mayo Clinic.His AHRQ grant focuses on how to optimize prevention and health care management for patients with diabetes.On average, patients with diabetes often have coexisting medical conditions, with depression, hypertension, and hyperlipidemia (high levels of ch*lesterol or other lipids) being most common.

"When patients aren't achieving guideline-recommended levels of control of sugar or ch*lesterol, our reflex is to intensify care by offering more treatment.This intensification leads to more visits, tests, side effects, and costs.Some end up being superfluous," says Dr.Montori.

"They don't fit with the patient's goals.Sometimes, the solution is minimally disruptive medicine that allows patients to pursue their dreams," he explains.For an elderly man with diabetes, depression, and a disability who desperately wants to maintain his independence, Dr.Montori is working to reduce the man's workload as a patient and simplify his routine to focus on preventing another stroke."We need more care that fits the patient," Dr.Montori a*serts.AHRQ's investments in exploratory research projects like these, as well as projects that develop new methods and data resources, will help researchers improve our understanding of complex patients and help personalize care for better results.

A decree published last night for the return of thousands of properties seized in ’36, just hours before an Iftar of the Prime Minister with representatives of religious minorities.The beneficiaries are Greek-orthodox Christians, Armenians, Jews.Roman Catholics do not fall within the recognized minorities.The Prime Minister’s hopes: end to era of discrimination.

This is Erdogan’s second surprise reserved for the old establishment of the Turkish Republic after the recent decapitation of the heads of the armed forces and the return of the primacy of politics over the military.

The publication of the draft-law on the restitution of property took place yesterday, just hours earlier than the traditional Iftar [the dinner-party that celebrates the end of the Ramadan fast] which the representative of the non-Muslim religious foundations, Lakis Vingas, held last night with the Prime Minister guest of honour.

The publication of the draft-law is a real «coup de theater»: it will return all property to religious foundations that the Turkish administration with various subterfuges has seized in the past, after the census of 1936.Non-Muslim religious foundations means those recognized by various international treaties signed by Turkish Republic after 1923.

The decree has been published within a few days of Bartholomew I’s request for the return of unjustly usurped properties to minorities.In his campaign to see the return of certain properties of the Greek-orthodox communities, Bartholomew I had approached various European forums.

The decree provides:

1) the restitution of property as they were surveyed and registered in 1936 and subsequently confiscated from the religious foundations by the various administrations of the Republic of Turkey;

2) the return of the management of cemeteries belonging to non-Muslim foundations, which have been improperly sold to various towns and municipalities;

3) the restitution of undefined deeded property (such as monasteries and parishes), which were never recognized as legal entities by the Turkish Republic.

4) In the event that these properties have been sold or disposed of in various ways by the Turkish state parties, the Minister of Finance of the Republic of Turkey will establish with the owners a just compensation.

Interested parties are invited to submit the relevant documentation to the Directorate General of Foundations within 12 months.

It should be noted that the last law of the Turkish parliament voted on February 20, 2008, challenged and never accepted by opposition did not provide any of these regulations.What remains to be determined is the fate of mazbut properties (the so-called «occupied» properties) in which management, administration and property pa*sed to the Turkish state.

According to an initial calculation, the decree provides for the restitution of 1000 properties to the Greek-orthodox Christians, 100 to the Armenians, numerous properties to the Chaldean Catholics and also to the Jews.

Nothing is expected for the Roman Catholics as they do not fall under the Treaty of Lausanne.But according to observers, the pa*sage of the decree gives hope.

The decree has provoked positive reactions from all minority representatives.The director of the non-Muslim foundations described it as «a step of great importance and great historical content», the lawyer for minorities, Dr.Kezmpan, described it as a great revolution, after the liberation from the military dominance» .Another lawyer, Dr Hatem said that finally «the wrong done to the Church is restored.»

In recent years the EU has always asked Turkey to take steps to remove discriminatory laws against religious minorities.And in some cases the European Court for Human Rights has condemned the Turkish state to return property or compensate the former owners.

At the Iftar yesterday, Erdogan said: «Like everyone else, we also do know about the injustices that different religious groups have been subjected to because of their differences Times that a citizen of ours would be oppressed due to his religion, ethnic origin or different way of life are over».

Sunday's interesting case - Two emaciated adult dogs "When you consult 10 vets, you will get 10 different opinions in most cases," I said to the mother and daughter who consulted me on this fine Sunday morning, August 28, 2011.The daughter said the internet recommended me.My surgery was relatively far away for them."The first vet said she would cut away the "three" mouth growths," the daughter said angrily."She cut one and sent it to the laboratory.The report said it was not a cancerous growth.So she told me to apply the cream onto the growths in the mouth as they were infections.She said the growths would then disappear."

Well, I had no comment.The histopathological report did not confirm that the growth was cancerous or not.

But the "three" oral tumours have grown much bigger.So they consulted Vet 2 nearby their home.But Vet 2 did not care to excise the growths and so the internet lead them to me on this fine Sunday morning when I was interviewing a fair young lady graduate with first cla*s honours in an Australian University.She wanted to volunteer on weekend to see practice to know whether she would like to study veterinary science.

"These oral tumours are likely to be cancerous," I stated on the fact that the growths grew fast over the last 3 months and the spread to the submandibular lymph nodes and down to the nearby ones.

"Feel this thick cord of 8 cm x 3 cm x 2 cm on either side of the neck from the submandibular lymph nodes," I asked the daughter and mother to place their thumb and forefinger onto the cord."These are likely to be spread of the cancerous cells from the mouth to the lymph nodes."

"What do you advise about the mouth tumours?Vet 1 should have removed them as promised." the daughter was quite unhappy as the tumours had metastasized and she had already spent $2,000.

I said: "Each vet has his or her own way of handling a case of mouth tumours.Vet schools may teach the vet to do a biopsy first.If the lab results find that the growths are not cancerous, then the owner is informed as to the next step.By then, some cancerous ones will have grown much bigger due to the delay.

"Unfortunately, tumours don't disappear even if they are not cancerous.In practice, it is best to cut them off when they are small.They interfere with the dog eating, being painful and ulcerated.And smelly.However anaesthesia in old dogs is highly risky and the dog may just die on the operating table.So the vet may not want to operate as deaths of dogs under his or her care badly damage his or her reputation."

The daughter asked: "What are the chances of survival?"

I said: "I estimated around 40% as your dog is emaciated.All skin and bones now.You can see the spine protruding."

The daughter said: "My dog eats a lot and will even eat the other dog's food.The vets don't believe when I say that."

"It is hard to believe.Any dog or person that eats a lot will put on weight.What do you feed?"

"Brown rice and potatoes," she said."The vet advised against dry dog food as she has skin problems.Her foot pads are licked red and so the vet thinks that my dog is allergic to dry dog food."

This is surprisingly the second emaciated dog I was presented on this fine Sunday morning.I had just hospitalised an emaciated poodle who had a painful mouth and screamed when I touched the mouth.Blood tests were taken to find out why.The lady owner had another dog with urinary stones.So no dry dog food for that dog.And similarly for this poodle.Skin and bones and protruding spines.

Back to the old Shih Tzu.What to do?
"Do you advise euthanasia?" the daughter asked me since I said that the Shih Tzu was at the end of his life-span of 10 years.Healthy ones may live up to 15 years but they usually live to 10-15 years in Singapore.

"The decision to euthanase is up to the owner," I advised.In advising euthanasia, some owners deem the vet to be a good for nothing.Only know how to advise euthanasia.

I said: "In many cases, it is a matter of economics too.These tumours can be cut off by electro-surgery.You may buy some time for the old dog."

"How long?"

"If they are cancerous, the tumours will recur around 2 months."

"What to do then?"

"It is best you and your mum take 3 or 4 days to think what to do," I said."If surgery is your option, I will operate but you know the chances of survival are around 40% and I have not done any blood test to know the dog's health, to save some money, since you had spent $2000 at the other vets."

"How much is the surgery?"
"I can only estimate.Around $500 - $1,000"
"My dog barks at me when I tell him that he will be put to sleep," the daughter said."So, I do not want to do it."

The owners went home to feed the dog on a good diet for the next 3-4 days, multivitamins and antibiotics.Surgery will be scheduled on Thursday morning at 10 am if she wants surgery.This is the type of high-risk surgery where fools go in where angels feel to tread.But is there any other choices?Chemotherapy will not work and I don't want the owners to get false hopes.Neither will surgery if there is recurrence.But there is no other choice.It was Hobson's choice.

My teeth are horrible....?

My teeth are horrible....? Ok I am 16 and I basically have horrible teeth.I haven't been to the dentist in over a year because I guess its not on the top of my family's priority list.Yeah I also really really need braces but I have only been to the orthodontist once and that was 3 years ago.I also have a horrible time with brushing my teeth too, I am lucky if I brush them once a day but I have gone without brushing them for 2+ days before.This is beacuse brushing my teeth makes me cough and gag really bad...Ugh I feel horrible and I know that I what I am doing is terrible.Any suggestions how I can talk my parents into taking me to the dentist (we have dental insurance) or how I can talk them into how important braces are or with brushing my teeth in general??I have already talked to my parents and I have even printed them off articles but they are just like yeah ok eventaually we will get to it...

owch they look painful, i had a tooth sticking up in the middle of my mouth and i thought that was bad before they removed it.My advice would be to talk to them again, just tell them how much it means to you, and mention that once you have been to the dentist and orthodontist and they have signed the papers they don't have to come with you any more (once you get the appointment for the braces and they have signed everything there's nothing that they need to do) Tell them that it ill take about a hour of their time and then you will be happy.(i didn't go to the orthodontist for about 6 years as first time i went they told me i couldn't be treated as i had too many baby teeth left) just keep on at them until they give in and tell them that it is impossible to brush them as they are (i know how you feel cause i found it hard to brush before i got my braces) With the coughing and gagging is it because you have to push the tooth brush in too far (if so i can't help that only a dentist can) or is it because the toothpaste if horrible and gets down your throat (if that's the case just ask your parents if you can try a different brand, even if it is just for you to use)

The Association of Indian Management Schools (AIMS) today strongly criticized the All India Council for Technical Education (AICTE) for putting «unreasonable» restriction on institutes imparting PGDM courses and called upon the a*sociation members to intensify their fight against the regulatory body.

It may be noted AIMS has already challenged the AICTE notification on PGDM schools in the Supreme Court and the apex court has granted two interim stays -one for the year 2011-12 and the other for 2012-13 as the writ petitions are still pending for disposal.Referring to the AICTE notification on 28th December, 2010, Prof J Phillip, founder president AIMS and president, Xavier Institute of Management and Entrepreneurship, Bangalore, said, the regulatory body has shown the tendency of being over-restrictive in its approach, often leading to inflexibility, mediocrity and lack of innovation.

Speaking at the inaugural session of 23rd Annual Management Education Convention of AIMS, organized by Regional College of Management, a city based autonomous BSchool, here, Phillip said a ma*sive educational and advocacy effort is needed to bring in proper balance in the approaches of the regulatory bodies.

He sought the partnership of organizations like Confederation of Indian Industry (CII), Associated Chamber of Commerce and Industry (ASSOCHAM), Federation of Indian Chamber of Commerce and Industry (FICCI) as well as the liberal minded people to fight the negative mindset of the regulatory bodies.

The AICTE notification, which is strongly opposed by AIMS, include provisions that admission to all PGDM courses shall be done through common entrance test such as Common Admission Test (CAT), Management Aptitude Test (MAT) or examination conducted by the respective state governments for all institutions other than minority institutions from which Joint Management Entrance Test, Xavier Aptitude Test, ATMA are excluded and fees charged for the PGDM, PGDM (executive) and Post Graduate Certificate in Management (PGCM) programmes shall be approved by the fee fixation committee of respective state governments.

Besides these, AIMS has contested four other provisions of the notification in the Supreme Court.

Among others, Orissa governor, MC Bhandare and Santrupta Mishra, CEO, Carbon Black and Group HR, Aditya Birla Management, delegates from abroad and within the country attended the convention.

4 Basic Baseball Drills for Kids

Coaching middle school baseball can be challenging, because you'll likely have a mix of players with different ability levels.Some players might be just beginning, while others might have been playing for years.You'll also encounter physical differences among players who have not yet started puberty, and those who have already developed physically.Keep these factors in mind, and conduct practice drills appropriate for players of all experience and maturity levels.
Long Toss Drill
The long toss drill builds your players' arm strength.Pair each player with another of similar ability.Have one group of partners start on the outfield foul line, about five yards apart, and the other group in the outfield, about 30 yards off the foul line.After lining them up, have your players play catch.Have the group in the outfield back up five yards after every set of 10 throws.Make sure they throw the ball on a line, not in long, rainbow-type arcs.
Partner Ground Ball Drill
The partner ground-ball drill teaches players how to field grounders properly.After the long toss drill, have the players in the outfield move within about 5 yards of their partners.Tell all your players to a*sume an appropriate ground-ball fielding position, and have them roll the ball back and forth.Emphasize that they should field the ball in front of their body with both hands.You can also have your players practice fielding grounders with their backhand and forehand.
Fly Ball Communication Drill
This drill teaches your players how to catch fly balls and communicate with their teammates during a game situation.Position two cones about 20 yards apart in the outfield.Divide your players into two groups, and have one group form a line behind each cone.Stand about 30 yards in front of the players, and repeatedly throw balls high in the air between the cones.For each throw, the players at the front of each line should move to catch the ball.They must communicate with each other to avoid a collision; once one player calls the ball by yelling "I've got it," the other should yell "take it" and move out of the way.
Rapid Front Toss Drill
The rapid front-toss drill gives your players a lot of swings with the bat in a short time, and helps improve their bat speed.Position a screen 15 to 30 feet in front of home plate.Have one player bat at a time, and the others scatter across the outfield.Stand behind the screen with a bucket of balls.When the batter is ready, toss a ball underhanded into the strike zone.The batter should try to hit a hard line drive.After he swings, tell him to reset as quickly as possible, then immediately toss another ball.Continue for 10 to 20 swings.

Airports : a holiday destination in themselves

I don’t get to travel a lot in my field of work, except when I’m on holiday.I’ve always enjoyed being at an airport, as a kid, it was seeing all the planes, which if I’m honest is still fun for me.But as I’ve grown up, airports fascinate me for many reasons, not just for the planes.

So I’m on holiday currently, visiting Toronto and Boston.I flew British Airways via Heathrow and to my delight, I had a four hour layover at London.Since I’m traveling alone this time (hate it, never like traveling without my wife), I got to indulge in my airport self-entertainment wh*leheartedly.

So the reasons why I like hanging out at airports are

1.The planes

First and foremost, it’s the airplanes.I love sitting and staring at them.Taking off, landing.I prefer watching planes land, it’s like a ma*sive steel apartment building falling from the sky.Watching the plane approach the strip, make last minute adjustments and then when the wheels hit the tarmac, I wait for the plume of black smoke.What an adrenaline rush it must be for the pilots!

2.Food

Whenever I travel abroad and meet friends or family, they insist that we try «good» food at «good» restaurants.I can’t really satisfy my cravings for trashy junk food.So, when I’m alone at an airport, I can go nuts.If you’re ever pa*sing through an airport and come across a chubby Indian gleefully tucking into a burger king meal, that would be me.

3.The bookshops

Undoubtedly my favorite activity.Yes yes, we have bookshops back home but the lure of airport bookshops is irresistible.I don’t end up buying paperbacks, it’s usually a foreign edition of GQ or Vogue or Esquire.In short, cla*sy literature.

Also, my wife loves the airport bookshops as well.But for entirely different reasons.She’s not interested in the books or the magazines.She usually ends up buying silly unnecessary candy near the cashier.She’ll be like «I’m telling you, you don’t get these sour skittles ANYWHERE», or something like «I’m buying these orange tic-tacs for my mum».Anyway, the candy usually finds it’s way to the bottom of her mega-bag, only to resurface years later during some boring sight-seeing trip on some other vacation.And then she’ll gloat «And to think, you weren’t going to let me buy these skittles, who’s loving them now?».Honey, I’d be loving the skittles if you didn’t keep trying to pa*s me all the disgusting watermelon flavored ones no one likes.

4.The other shops

My favorite shops are the ones where they try to sell you stuff you never needed till you saw it.Products like an alarm clock that jumps off the bedside table and runs away (genius), tempur-pedic neck pillows, weird space-age universal chargers, wireless cellphone chargers, headphones of all shapes and sizes and even disposable underwear (yes, you read correctly).

There’s also all the perfume stalls (makes me sneeze, so I stay away) and the duty free booze (usually to pick up single malt for home, after the cursory «Dad, Which one should I buy?» phone call).

5.The people

Oh how I love to stare.I love people-watching.What they’re wearing, who’s listening to music, who uses a Mac, who’s a PC guy, what earphones they’re using etc.You can tell a lot about a person by how they conduct themselves at an airport.

I’m always envious of people who manage to travel gracefully.There are a few in every flight.They’ll be dressed so well, not a crease on their button down blue shirt, impeccably well fitting jeans and beautifully polished shoes.These are the ones that never carry any hand luggage, all they have is a neatly folded NewYorker in their hands (pretentious pricks).I wish I could travel like that.When these seasoned, slick travelers stroll past me down the aisle, I’m the guy who’s usually trying to straighten my completely accidental bed-head hairstyle and scrape off dried spit from my cheek.I feel fat.

So on this flight to London I just took, there was this lady in her fifties, she was really fit (didn’t look a day over forty), who was wearing a white knit turtleneck and beige pants.She was carrying this wonderful tan soft leather bag by Tod’s and loafers also probably by Tod’s (it’s my story, I’m going to promote the brands I love.Deal with it).When she got off the plane, she looked like she stepped off a spa.No sign of that 9 hour flight fatigue, no curry stain on her white turtleneck.There’s a special place in hell reserved for such people.

There are so many more reasons but I think you get the gist.This is why sometimes I wish I was a busy businessman who had to travel a lot for work.I envy my friends who do, though they seem to hate it.I guess not everyone wants to be like Clooney in в

Review: A Lily Among Thorns by Rose Lerner

Rose Lerner’s stunning prose and keen eye for detail come together to create a unique, spellbinding novel

Serena recognizes Solomon immediately.She used his money to buy her way up in the underworld, eventually becoming mistress to several high-flying men before going straight and opening an inn.She wants to return the favor, but having him around threatens the barrier she’s built around her heart in order to survive.

I’m not sure I can find words to express how much I love Rose Lerner’s prose.A LILY AMONG THORNS is only her second novel; her first, IN FOR A PENNY, came out last year and immediately became one of my favorite books of 2010.

I didn’t think it would be possible, but Rose Lerner’s second novel is even better than her debut.Her intricately drawn characters break romance-novel norms.This is no typical gentleman/courtesan story.Selena is a lady who chose to leave her family’s wealth in pursuit of love and lust.And Solomon is a parson’s son who chose to become a tailor andin today’s languagea fashion designer.

Their differences in cla*s, and the ways they struggle against the constraints of society’s expectations of them, provide oodles of humorous conversationslike this one, which takes place long before the two of them are intimate, in a scene where Selena’s aristocratic father arrives at the inn to browbeat her for taking up with someone like Solomon.Solomon responds:

«Your daughter has done what none of your blood has done since the Conquestkept an honest roof over her head with the fruits of her own honest labor!And you come here and insult her under it.Are you not ashamed?»

Lord Blackthorne’s lips were white.«If you were a gentleman I would call you out for that.As it is, you are fit only for horsewhipping.»

«That’s just as well, for I should certainly not meet you,» Solomon bit out.«Dueling is an outmoded and barbaric custom, fit only for killing off the stupider members of a thoroughly useless cla*s.»

Lord Blackthorne had been angry.Now, he was simply astonished.«Is he this prosy between the sheets?» he asked his daughter.

Her smile was cold, but her eyes were dancing now.«Oh no, Father.There he is pure poetry.»

I highlighted so many pa*sages that made me smile or made my heart clench.Rose Lerner is masterful at bringing out the details that make characters human, in a way that reminds me of Judith Ivory and Meredith Duran.I highly recommend this novel and can’t wait for her next novel.

sedation dentistry question?

sedation dentistry question? I'm terrified of going to the dentist - because of this my teeth have been suffering.Well as of friday i have had the worst tooth ache ever, it hurts if i close my mouth and my teeth touch that tooth.I havent been to the dentist since i was a kid (im 20 now) I was wondering about sedation dentistry...is it safe?Also i don't think we have dental insurance, so how much will it cost?I don't know what to do, im suffering with this toothache, but im embarra*sed to go to a regular dentist, and i fear the pain..

I feel your pain, as i just went to the dentist for the first time in 14 years last Thursday (after a traumatizing dental experience as a kid), and lucky me, I found myself in the dental surgeon's office the next day getting an abscessed tooth pulled.All the while, my dental phobia is well on it's way to being out of my life.As far as sedation goes, some dentists will give you two pills to take; one an hour before your appointment, and one when you get to the office.This will pretty much put you in a relaxed state of mind, not having a care in the world.Some also offer laughing gas, IV sedation, etc etc.Mind you, for IV sedation and such, your toothache will be nothing compared to your bill.Even when they pulled my tooth, they used Novocaine, and I was so numbed up, I was actually amused by slapping myself before the doctor came to get the work done!If I were you, I'd just take a deep breath, call up the office, let them know of your fear, and go on in.Just tell your dentist about your fear, and if he's a good dentist, he will be understanding.More than likely, he's going to just look at the tooth that's been bugging you, take a few X-Rays, and that'll be it.Especially with your toothache, I definately wouldn't let that go.If it's abscessed, and you let it continue on it's course, it can become life threatening.Good luck to you, I hope you feel better soon.=EDIT= I looked up IV sedation, and you're looking at $250.00 for that alone, the simple tooth extraction being around $80 if you needed that (may or may not.)
